What helps us respond quickly

You don’t need everything below — send what you have and we’ll follow up for the rest.

  • Participant name (or initials) and NDIS number
  • Plan management type — NDIA, plan-managed or self-managed
  • Plan start and review dates, and the funded support areas
  • The supports being requested and the participant’s goals
  • Location / suburb and how often support is needed
  • Any risk or safety information, and whether a Behaviour Support Plan is in place
  • Confirmation the participant (or their representative) consents to the referral
What happens next
